What Prostate-Specific Antigen Is

PSA stands for Prostate-Specific Antigen, a protein made by cells of the prostate gland. Despite the name it is an enzyme: its natural job is to thin the semen after ejaculation so that sperm can swim freely.

Small amounts leak into the blood, which is why a simple blood test can tell you something about a gland deep in the pelvis. The result is reported in nanograms per millilitre (ng/mL), and the test is the most widely used blood screening tool for prostate cancer. It is not specific to cancer, and the name means the protein comes from the prostate, not that it signals disease.

Reading a PSA Value: What Counts as Normal

For many years 4 ng/mL was treated as the dividing line between normal and suspicious. Doctors now read the figure against age, because PSA rises slowly with age on its own: the same reading means more in a 48-year-old than in a 72-year-old, and several guidelines use age-specific ranges rather than one cut-off.

A raised PSA does not mean cancer. An enlarged prostate (benign prostatic hyperplasia), a urine infection or prostatitis can all raise it, and so can recent ejaculation, long cycling, catheterisation or a prostate biopsy in the days before the sample. A rectal examination may have a small effect, so many doctors still take blood first.

Free PSA, Trends and Follow-Up Tests

Reading the free PSA ratio

When the total PSA lies between 4 and 10 ng/mL, labs often add a free PSA test. The share of free PSA is lower in cancer, so a value below about 10 per cent raises suspicion, while a value above about 25 per cent points more towards a benign cause. Urologists use the ratio, along with other findings, to decide who needs a biopsy or an MRI first.

A trend beats a single number

A PSA that doubles within a year worries a doctor more than a stable, mildly raised reading, which is why repeat results are compared. After surgery to remove the prostate the level should fall to almost zero, and a later rise is usually the first sign of recurrence, often before a scan shows it.

PSA Testing in India: Who Should Consider It

India has no national screening programme for prostate cancer, and international guidelines advise against routine PSA testing of healthy younger men, since a false alarm can lead to an unnecessary biopsy. Many urologists suggest discussing a baseline test at about 50, and earlier if a father or brother had prostate cancer.

Symptoms change the picture. Frequent night-time urination, a weak stream, difficulty starting or blood in the urine justify a PSA and a urology consultation at any age. The test needs no fasting and a report normally comes within a day or two at an accredited lab. Ask your doctor whether you also need an USG of the prostate and bladder.

Some practical points before you go for the test: tell the lab or doctor about recent illness, a catheter, or a procedure; avoid heavy cycling and ejaculation for about two days; and ask whether the lab uses the same method as before, since different methods can give slightly different values. When a doctor suspects a problem, the next steps often include a digital rectal examination, an MRI of the prostate and, if needed, a biopsy guided by imaging. Men who take medicines for prostate enlargement should tell the doctor, because some of them lower PSA and change how the result is read.

The Other PSA and a Plan for Your Report

The same letters stand for Public Service Announcement, a short notice in the public interest issued by a broadcaster, ministry or NGO. In medical writing, however, the acronym almost always means the prostate protein.

Treat one number as the start of a conversation, not a verdict. Symptoms, family history, a repeat reading and the free PSA ratio together decide more than the first figure on the page, and a urologist weighs them all. Bring earlier reports so the trend can be compared.

PSA is a useful blood signal, not a diagnosis. Take the report and your family history to a urologist instead of acting on the number alone.

PSA Full Form: Common Questions

What is the full form of PSA?

PSA stands for Prostate-Specific Antigen, a protein made by the prostate and measured in blood in ng/mL. A high value can point to cancer, but also to enlargement or infection.

What is a normal PSA level?

A reading below 4 ng/mL has traditionally been called normal, but many labs now give age-specific ranges because PSA rises with age. If it is between 4 and 10, doctors often check the free PSA percentage.

Can a high PSA mean something other than cancer?

Yes. Enlarged prostate, prostatitis and urinary infection raise PSA, as do ejaculation, long cycling, catheterisation or a recent biopsy. Doctors often repeat the test after some weeks.

At what age should men in India get a PSA test?

There is no national schedule. Many urologists suggest discussing it at about 50, earlier with a family history, and at any age if urinary symptoms appear.

Does a PSA test require fasting?

No fasting is needed. Labs often ask you to avoid ejaculation and strenuous cycling for about 48 hours before the test.

Does a high PSA mean I need a biopsy?

Not always. The doctor looks at the trend, free PSA, symptoms and often an MRI before deciding on a biopsy.
