OTC full form and what counts as over the counter

OTC stands for Over the Counter. It covers medicines a buyer can purchase directly from a pharmacy without a doctor's written prescription: paracetamol, antacids, simple cough syrups, oral rehydration salts (ORS), antihistamine tablets and topical antiseptics.

The phrase describes the sale, not the strength of the drug. A medicine is sold across the counter because the risk of self-treatment at that dose is considered low and the condition is easy for a layperson to recognise. The same molecule can be prescription-only at a higher dose or in a different form.

India's Drugs and Cosmetics Rules, 1945 work the other way round. They do not set up a formal OTC list. They list the drugs that must be sold only on prescription, and anything outside those schedules is in practice sold without one.

Which Indian medicines require a prescription

Schedule H drugs carry the Rx symbol and may be sold only on the prescription of a registered medical practitioner. Schedule H1 adds a stricter layer: the chemist must keep a register of the sale, including the patient and prescriber details, and the drug carries a warning box. Many antibiotics and some anti-TB medicines sit here.

Schedule X covers narcotic and psychotropic substances, where the chemist must keep the prescription copy for two years. Schedule G drugs carry a printed caution that the medicine should be taken only under medical supervision.

Practically, that means antibiotics, antidepressants, blood pressure and diabetes tablets and sedatives are not OTC here. Emergency contraceptive pills, by contrast, can be bought without a prescription in India, which surprises many buyers.

What actually happens at the chemist's counter

A drugstore needs a licence, and sales are meant to be made under the supervision of a registered pharmacist. The brand on the shelf is rarely the only option: a generic version of the same salt, sold under its chemical name, is usually cheaper and works the same way.

Ask for the salt, not the brand. Paracetamol 500 mg is the same molecule whether it is sold as Crocin, Dolo or a shop brand. Chemists often hand over a different brand when a pack is out of stock, and knowing the salt means you can check that the substitute matches.

Read the strip before you leave. The composition, expiry date, batch number and maximum retail price are printed on it. Selling above the MRP is not legal, and a strip with no expiry date should be left at the shop.

Price varies widely too. Government Jan Aushadhi stores sell quality-checked generic medicines at a fraction of branded rates, and their outlets stock commonly used items such as oral rehydration salts, paracetamol and antacids.

The limits of self-medication

OTC does not mean harmless. Paracetamol overdose is a well-known cause of acute liver failure, and the usual adult ceiling is about 4 g a day from all sources combined, including combination cold remedies that already contain it. Read the label of every product you take together.

Some cough syrups and cold remedies contain sedating antihistamines, which cause drowsiness and are a hazard for anyone who drives. Painkillers of the NSAID group, such as ibuprofen and diclofenac, can irritate the stomach and strain the kidneys with regular use.

Any symptom lasting more than a few days, high fever, chest pain or breathlessness belongs with a doctor rather than the counter. Pregnant women, people with liver, kidney or heart disease and older adults on several medicines should ask before taking even a simple tablet.

Children's doses are weight-based, so the adult pack is not a shortcut. For infants, oral rehydration solution and paediatric formulations are the safer first response, and a paediatrician should be called early rather than late.

OTC in finance means something else

Outside medicine, OTC describes trades settled directly between two parties without a stock exchange, as in over-the-counter bonds, derivatives and currency deals. If a search result mixes the two, the surrounding word, medicine or market, tells you which meaning applies.
