How autism spectrum disorder is defined

The DSM-5, published in 2013, merged autistic disorder, Asperger's disorder, childhood disintegrative disorder and pervasive developmental disorder not otherwise specified into one heading, Autism Spectrum Disorder. ICD-11, the WHO classification that member states began using from 2022, also uses a single spectrum category.

Two core features define it: persistent differences in social communication and interaction, and restricted or repetitive patterns of behaviour, interests or activities. Clinicians describe how much support a person needs, from level 1 (needing support) to level 3 (needing very substantial support).

Autism is neurodevelopmental. It is present from early childhood and continues into adulthood. It is not a disease, and there is no blood test for it. Diagnosis rests on developmental history, observation and standard tools such as ADOS-2.

Early signs and how diagnosis happens in India

Early signs include not responding to one's name by 12 months, no babbling or pointing, limited eye contact, delayed speech and loss of words a child once used, often around 18 to 24 months. Paediatricians in India increasingly look for these at routine check-ups.

Diagnosis is made by a developmental paediatrician, child psychiatrist or clinical psychologist through structured observation and parent interviews. Centres such as NIMHANS in Bengaluru and CMC Vellore, and many private child development clinics, offer assessment, although waiting lists in public hospitals can be long.

Parents' groups and NGOs such as the Autism Society of India and Action For Autism have pushed for earlier identification. The sooner a child is assessed, the sooner speech, occupational and behavioural therapy can begin.

Causes, evidence and the vaccine myth

Research points to a strong genetic component involving many genes, with some prenatal factors such as advanced parental age and preterm birth linked to higher likelihood. There is no credible evidence that parenting, diet or vaccines cause autism.

The vaccine claim began with Andrew Wakefield's 1998 paper in The Lancet, which was retracted in 2010, after which he was struck off the UK medical register. A large Danish study of over 650,000 children published in 2019 found no link between MMR vaccination and autism.

This matters in practice. Avoiding vaccines because of the myth exposes children to measles and its complications without lowering the chance of autism. Indian paediatricians advise completing the immunisation schedule on time.

Support, rights and daily life

Speech therapy, occupational therapy and structured behavioural programmes, started early, can improve communication and daily living skills. Adults may need support with education, employment and independence, and many also manage anxiety or ADHD alongside autism.

The Rights of Persons with Disabilities Act, 2016 lists autism spectrum disorder as a specified disability, which brings reservation in government jobs and higher education for those who meet the benchmark, and rights to inclusive schooling. The National Trust Act, 1999 also covers autism in its welfare schemes. A disability certificate issued by a medical authority is the document used to claim these benefits.

Prevalence figures differ. The WHO uses a figure of about one in 100 children worldwide, while US surveillance reported about one in 31 eight-year-olds for 2022. India has no national registry, so the true figure here is uncertain.

Schools are expected to make reasonable adjustments, such as a quiet seat, visual schedules, extra time in exams and a trained special educator. Parents should ask the school in writing and keep the assessment report and disability certificate ready.

The other ASD: atrial septal defect

In cardiology, ASD is Atrial Septal Defect, a hole in the wall (septum) between the heart's two upper chambers. It is present from birth and is one of the commoner congenital heart conditions. It has no connection with autism.

Small defects may close on their own in childhood and may need only monitoring. Larger ones can strain the right side of the heart over years and are closed, usually with a catheter-delivered device or, in some cases, with surgery. Diagnosis is by echocardiography, and the decision rests with a cardiologist.

ASD describes how a person's brain processes the world, and it is not a defect to be fixed. With early support, suitable schooling and reasonable adjustments at work, many autistic people in India study, work and live far more independently than families first expect.

ASD Full Form: Common Questions

What is the full form of ASD?

ASD stands for Autism Spectrum Disorder, a neurodevelopmental condition affecting social communication and behaviour. In heart medicine the same letters stand for Atrial Septal Defect, which is unrelated.

Is autism a disease that can be cured?

No. Autism is a lifelong developmental difference, not an illness. Therapy can build communication, learning and daily skills, and many autistic adults live independently.

At what age can autism be diagnosed?

A reliable diagnosis is often possible by 18 to 24 months, and sometimes earlier with an experienced specialist. Many Indian children are identified later, so parents should seek an assessment when speech or response to name seems delayed.

Do vaccines cause autism?

No. The 1998 paper that started this claim was retracted and its author struck off the register. A 2019 Danish study of over 650,000 children found no connection between MMR vaccination and autism.

What support is available in India?

The Rights of Persons with Disabilities Act, 2016 lists autism as a disability, giving access to reservation and accommodations in education and jobs for those who qualify. Early intervention centres and therapy services are available in most large cities.

What is atrial septal defect?

It is a hole between the heart's two upper chambers, present from birth. Small ones may close by themselves, while larger ones are closed with a device or by surgery on a cardiologist's advice.
