Effects usually begin within an hour of swallowing a tablet and fade over several hours, followed by a low, tired period.

Why MDMA is illegal in India
MDMA is listed as a psychotropic substance under the Narcotic Drugs and Psychotropic Substances (NDPS) Act, 1985. Possession, production and sale are all offences, and penalties scale with the quantity found, from a short jail term for small amounts to very long sentences for commercial quantities.
Enforcement is concentrated around airports, party venues and festival seasons in cities such as Mumbai, Goa and Hyderabad. Because tablets are unregulated, a major risk is what else is in them: caffeine, methamphetamine, mephedrone and other substitutes are often found in seized batches.
Health risks are real even when a sample is pure. Overheating and dehydration at crowded venues, or the opposite mistake of drinking too much water, can both become emergencies. Warning signs include a very high temperature, seizures, confusion and inability to pass urine. These need urgent hospital care, since there is no specific antidote. Heavy repeated use has also been linked to low mood, sleep trouble and memory problems.
MDMA in medicine and research
The main clinical work has been done by the Multidisciplinary Association for Psychedelic Studies (MAPS), which ran phase 3 trials pairing MDMA-assisted therapy with talk therapy for treatment-resistant PTSD. The published results showed larger symptom reductions than therapy with a placebo, though the trials were criticised over design and reporting.
Australia went first: from July 2023, authorised psychiatrists there can prescribe pharmaceutical-grade MDMA for treatment-resistant PTSD, the first country with such a legal pathway. It is not a take-home medicine. The protocol places the patient in supervised sessions lasting several hours with trained therapists present.
In the United States, an advisory panel to the FDA voted against the application in June 2024, and the regulator declined to approve it that August, asking for more study. In India there is no approved medical use, and any research would need approval from the drug regulator.
