How the test works

ESR stands for Erythrocyte Sedimentation Rate, often called the sed rate on Indian reports. Blood is drawn, mixed with an anticoagulant and kept upright in a narrow Westergren tube. After exactly one hour the technician reads how far the red cells have fallen, in millimetres.

Nothing forces the cells down. The reading depends on how readily red cells clump into stacks called rouleaux, which in turn depends on the proteins in the plasma around them.

Why inflammation speeds it up

During infection or inflammation the liver releases fibrinogen and other large proteins into the blood. These reduce the negative charge that normally keeps red cells apart, so the cells stack like coins and settle quickly. In healthy plasma the cells stay apart and sink slowly.

That is why ESR is called a non-specific marker. It rises with inflammation anywhere in the body, from tuberculosis, rheumatoid arthritis and lupus to a heart attack, a tumour or a kidney infection, without showing which one.

Normal numbers and what counts as high

Commonly quoted ranges are about 0 to 15 mm per hour for men and 0 to 20 mm per hour for women, rising with age. Pregnancy and anaemia raise it as well. Labs print their own range on the report, and that is the figure to read against.

Some conditions push it much higher. In adults past 50 with a new headache or shoulder pain, giant cell arteritis and polymyalgia rheumatica can drive ESR very high, and arteritis is treated urgently because it can threaten eyesight. Multiple myeloma can also give extreme readings, while sickle cell disease and polycythaemia can keep the value low even when the patient is unwell.

ESR versus CRP: which one when

CRP rises within hours of an insult and falls within days, so it follows an infection or flare almost in real time. ESR climbs over a day or two and stays up for weeks because it depends on plasma proteins that take time to clear. It is better at showing that a process has been running than at showing how active it is today.

Anaemia alone can lift ESR without active inflammation, so a high ESR with a normal CRP may point to anaemia, pregnancy or a plasma-cell disorder and not to infection. Indian labs often run the two together because each can mislead on its own.

Where ESR still earns its place

ESR needs only a tube, a stand and a clock, so it is inexpensive and available even at small district labs alongside a complete blood count. Doctors order it to look into unexplained fever, to follow conditions such as rheumatoid arthritis, and in some tuberculosis and bone infection work-ups.

ESR moves slowly. A value that stays raised after an infection clears does not always mean the infection is still there, since the proteins take weeks to drain away. The trend across visits tells more than any single reading.

A raised value never diagnoses anything by itself. It is a prompt to look further, with CRP, imaging, cultures or specific antibody tests. A normal value in a patient who looks ill deserves the same attention as a high one in a patient who feels fine.

For the test itself, tell the lab about any recent illness, pregnancy or medicine such as steroids, since these can change the reading. The sample should be tested within a few hours of collection, and a sample left standing too long can give a misleading value. If you repeat the test, use the same lab where possible, because methods and reference ranges differ.

ESR is one of medicine's cheapest measurements: how far red cells sink in an hour. It will not name your illness, but a rising or falling trend, read beside your symptoms, gives your doctor something concrete to follow.
