The Three Diseases DPT Prevents

DPT stands for Diphtheria, Pertussis and Tetanus. Diphtheria is caused by the bacterium Corynebacterium diphtheriae. It forms a grey membrane across the throat that can block breathing, and its toxin can damage the heart and nerves.

Pertussis is whooping cough, caused by Bordetella pertussis. Infants cough in violent bursts followed by a gasp, and a baby under six months may stop breathing or develop pneumonia. It spreads easily within families.

Tetanus comes from Clostridium tetani spores in soil and dust entering a wound, including the umbilical stump of a newborn. The toxin locks the muscles, and lockjaw is the classic sign. It does not spread from person to person, so only vaccination gives protection.

Where DPT Sits in India's Immunisation Schedule

Under the Universal Immunisation Programme, the first three doses of DPT come inside the pentavalent vaccine, which combines DPT with hepatitis B and Hib. They are given at six, ten and fourteen weeks, free at government facilities.

A DPT booster follows at sixteen to twenty-four months. A second booster, given as DT, comes at five to six years, and Td is given at ten and sixteen years. Mission Indradhanush and its intensified rounds were launched to reach children who had missed doses.

Pregnant women receive Td doses during antenatal care, which passes protection to the baby in the first months when whooping cough is most dangerous. Private clinics may use their own combination brands, but the schedule stays the same, and the immunisation card should be carried to every visit.

Other vaccines on the same card include BCG at birth, given against tuberculosis (BCG), and the MMR or measles-rubella dose at nine months. Your ANM or the nearest PHC can tell you which sessions run on which day, since village immunisation is usually held on fixed days of the month.

Whole-Cell and Acellular: Why the Reaction Can Differ

India's government programme uses a whole-cell pertussis vaccine, made from killed whole bacteria. It gives a strong immune response at low cost. Many private clinics offer acellular vaccines (DTaP), made from purified bacterial parts, which usually cause fewer reactions but cost more.

Expect a sore arm, mild fever and a day of irritability after the injection. A lump at the site can last a few days. High fever, a seizure or unusual crying for hours is uncommon but should be shown to a doctor. Paracetamol in the dose the doctor advises helps with fever, and fever after a jab usually means the immune system is responding.

Parents often ask about the injection site. In an infant the DPT-containing injection is given in the outer thigh. Holding the baby close and breastfeeding soon afterwards usually settles the crying, and a cool, clean cloth over the site can ease the soreness.

Precautions and Genuine Contraindications

A mild cold or runny nose is no reason to postpone a dose. A dose is delayed only when a child is seriously ill at the time, and the next visit should be booked straight away.

A severe allergic reaction to an earlier dose, or encephalopathy within seven days of a pertussis-containing dose, changes the plan. The doctor will then usually give DT, which leaves out the pertussis component.

Most clinics ask you to wait about fifteen minutes after the injection so that a rare immediate reaction can be handled. Bring the immunisation card, because it is also the record needed for school admission.

Missed a Dose, Catch-Up and Adult Boosters

The schedule is forgiving. If a dose is missed, the course continues from where it stopped rather than starting again, usually with a gap of at least four weeks between the primary doses.

Adults need tetanus cover too: a booster roughly every ten years, and an extra dose after a dirty or deep wound if the last one was more than five years ago. Many doctors give Tdap once in adulthood, and during every pregnancy, to protect the newborn from whooping cough.
