Who an ASHA Is and Where the Scheme Came From

ASHA stands for Accredited Social Health Activist. She is a woman chosen from her own village or urban slum, trained, and given the job of connecting families to the public health system. The programme started in 2005 under the National Rural Health Mission and now sits under the National Health Mission.

She is not a doctor and not a nurse. Her work is to reach people who would otherwise miss out: a pregnant woman who has not registered, a child who has missed a vaccine, a family that has never visited the sub-centre. India has roughly ten lakh ASHAs, with about one for every 1,000 people in rural areas and a similar arrangement in urban slums.

The word 'Accredited' matters. After selection she completes a series of training modules and receives certification, and that is what separates her from an informal helper.

Who Can Become an ASHA

The woman should be a resident of the village, married, widowed or divorced, and generally between 25 and 45 years of age, with education up to Class 8. Some states ask for Class 10 where enough candidates are available. She should be able to spare time for visits and be accepted by the community.

Selection involves the village health and sanitation committee and the block team, not a written exam. An ASHA is therefore someone neighbours already know. Training is given in rounds, with a field kit and a drug kit containing basics such as oral rehydration salts, iron and folic acid tablets and contraceptives.

A Working Week in the Life of an ASHA

Her tasks include registering pregnancies, counselling on antenatal check-ups, accompanying women for institutional delivery, home visits after birth, tracking immunisation, distributing iron tablets, spotting TB or malaria suspects and following up on malnourished children. During campaigns she also helps with pulse polio, deworming and fever surveys.

She keeps registers, and in many states a mobile app, to record what she has done. Her supervisor is an ASHA facilitator, and she works closely with the ANM at the sub-centre. She refers serious cases to the Primary Health Centre or the nearest hospital and often goes with the patient.

During the Covid-19 pandemic ASHAs did door-to-door screening, tracked home isolation and helped with vaccination drives, which brought wide public attention to their work for the first time.

Honorarium and Incentives

An ASHA is a community volunteer, not a salaried government employee. She earns performance-based incentives for tasks such as institutional deliveries, immunisation sessions and TB treatment support, plus a fixed monthly honorarium for routine work. Some states add their own top-up, so the total differs a lot from one state to another.

Unions of ASHA workers in several states have asked for a fixed salary, regular payment dates and social security. Anyone considering the role should check the current rates and the date of last payment in their own state before applying, because these figures change with each government order.

ASHA, ANM and Anganwadi Worker Compared

The ANM is a trained auxiliary nurse-midwife who gives injections and conducts antenatal check-ups at the sub-centre. The Anganwadi worker runs a centre under ICDS for nutrition and early childhood care. The ASHA is the community link who brings people to both.

The three are often mixed up in competitive exam questions. If the question asks for 'Accredited Social Health Activist', the answer is the ASHA, a health-department volunteer, not an Anganwadi or nursing post.

When the question is about maternal and child health in a village, the first name that comes up is usually the ASHA. Her title spells out what she is: trained, certified, and drawn from the community she serves.
