ACLS Full Form
Advanced Cardiac Life Support
What does ACLS stand for? A medical term, explained.
ACLS Full Form in English, letter by letter
ACLS is Advanced Cardiac Life Support, the emergency protocols for cardiac arrest and other life-threatening heart rhythms.
ACLS Full Form in Indian Languages
When a patient's heart stops or its rhythm turns dangerous, trained teams follow ACLS, Advanced Cardiac Life Support. The name is also given here in Hindi, Bengali, Tamil, Telugu, Marathi and Gujarati.
What ACLS adds to basic CPR
Advanced Cardiac Life Support is the layer of emergency care above basic CPR. Basic life support is chest compressions, rescue breaths and using a defibrillator. ACLS adds reading the heart rhythm on a monitor, managing the airway, giving drugs through a vein and following fixed algorithms for what to do next.
The treatment is based on international resuscitation guidelines, which are reviewed every few years by bodies such as the American Heart Association and the European Resuscitation Council. The Indian Resuscitation Council publishes guidance adapted to Indian conditions.
ACLS is team work. One person does compressions, one handles the airway, one gives drugs and one records times, and the team leader decides the next step. The aim is that tasks happen in parallel rather than one after another.
The cardiac arrest algorithm in practice
Shockable rhythms
For a patient without a pulse, the team starts compressions at 100 to 120 per minute and about 5 to 6 cm depth, attaches a monitor or defibrillator and checks the rhythm. Ventricular fibrillation or pulseless ventricular tachycardia gets a shock, then compressions immediately for two minutes before the next check. Adrenaline (epinephrine) 1 mg is given every three to five minutes, and amiodarone 300 mg, then 150 mg, if the rhythm keeps returning.
Non-shockable rhythms and the cause
In asystole or pulseless electrical activity there is nothing to shock. Compressions and adrenaline continue while the team searches for reversible causes: low oxygen, low blood volume, potassium imbalance, acidosis, tension pneumothorax, cardiac tamponade, a blocked heart artery or lung artery, and poisoning. After the pulse returns, care shifts to oxygen levels, blood pressure, temperature control and treating the cause.
Heart attack, stroke and abnormal rhythms with a pulse
ACLS also covers patients who still have a pulse. In a suspected heart attack, an ECG is done within minutes. If it shows ST elevation, the team activates the catheterisation lab or gives clot-dissolving drugs if angioplasty is not available quickly. Aspirin is given early unless there is a clear reason not to.
In stroke, the key detail is the time symptoms began. The FAST check (face drooping, arm weakness, speech difficulty, time to call) leads to a rapid CT scan, which decides whether clot-dissolving treatment is safe.
Abnormal rhythms with a pulse follow their own pathways: atropine or pacing for dangerously slow rhythms, and adenosine, drugs or synchronised cardioversion for fast ones.
Getting ACLS-certified in India
Doctors and nurses in emergency departments, intensive care, anaesthesia and cardiology are usually expected to hold ACLS certification, and many postgraduate programmes include it. Courses typically run for two days, with written and practical tests, at authorised training centres in cities and some district hospitals.
Certification is normally valid for two years, after which a refresher is needed. The interval exists because the skills fade quickly without practice, and badly done compressions are close to useless.
The drug list is standard worldwide: adrenaline, amiodarone, atropine, adenosine, calcium, sodium bicarbonate and magnesium for specific situations. In a busy ICU, the algorithm chart is usually posted on the crash cart so that the team can follow it under pressure.
BLS, ACLS and what a bystander can do
Basic life support is what any trained person can do: check for response, call for help, give high-quality compressions and use an AED. ACLS needs training, monitoring equipment and drugs, so it begins when a medical team arrives.
Early CPR and early defibrillation by bystanders decide most outcomes, and ACLS takes over after that. If you are not a healthcare professional, a BLS and AED course is the more useful one for you.
A ward code from start to finish
A patient on a medical ward collapses and does not respond. A nurse checks for breathing and a pulse for no more than ten seconds, shouts for help, presses the code button and starts compressions. Someone brings the crash cart with the defibrillator and an AED or manual monitor.
The team leader is usually the most senior doctor present. Roles are called out loud: compressions, airway and bag-mask, drugs, recorder. The monitor shows ventricular fibrillation, the leader asks everyone to stand clear, the shock is delivered and compressions resume at once for two minutes. The compressor is changed every two cycles because tired compressions are shallow.
Intravenous or intraosseous access is secured, adrenaline is given at the intervals in the algorithm, and the leader keeps asking what could have caused the arrest: low oxygen, low blood volume, potassium or acid imbalance, a blocked lung artery, fluid around the heart, a collapsed lung. If a pulse returns, the patient moves to the ICU for monitoring and an ECG. The recorder's timings are then used in the debrief, which is where most teams find what to improve.
In many hospitals the debrief ends with a note in the patient file and an entry in the code register, which is also what audits look at later.
Advanced Cardiac Life Support is the trained team's part of the response, and bystander CPR and early defibrillation are the other part. Anyone working in clinical care should certify and renew on schedule.
ACLS Full Form: Common Questions
What is the full form of ACLS?
ACLS stands for Advanced Cardiac Life Support, the set of protocols used by trained healthcare professionals for cardiac arrest and other life-threatening heart emergencies.
What is the difference between BLS and ACLS?
BLS is chest compressions, rescue breaths and defibrillation that trained laypeople can give. ACLS adds rhythm interpretation, drugs, airway devices and team-based algorithms, and it needs a certification course.
How long is ACLS certification valid?
Usually two years, after which a refresher course and assessment are needed. Hospitals often ask for current certification for emergency, ICU and anaesthesia staff.
What drugs are used in cardiac arrest under ACLS?
Adrenaline (epinephrine) 1 mg every three to five minutes is the main drug, with amiodarone 300 mg then 150 mg for rhythms that keep returning to ventricular fibrillation.
Who should learn ACLS in India?
Doctors, nurses and paramedics in emergency medicine, intensive care, anaesthesia and cardiology. Others are better served by a BLS and AED course.
Does ACLS include stroke care?
Yes. ACLS courses also cover acute coronary syndrome and acute stroke, with checks for onset time, a rapid CT scan and a decision on clot-dissolving treatment.
Is ACLS taught to medical students in India?
Most MBBS and nursing programmes teach basic life support, while ACLS is usually taken after graduation, during internship, postgraduate training or before working in an emergency department or ICU. Hospitals often arrange the course for new staff. Check the training centre's accreditation before paying a fee.
